Open a 300 x 25 canvas, then set these as your colors
Foreground: #D9D9D9
Background: #525252
Select your gradient tool and set it to "Reflected", now take it, stick it about in the middle of the canvas and then drag it down (be sure to go straight, so you don't get a funny reflection, try holding shift) to the end of the canvas and maybe a bit past. Be sure to experiment with this until you get something you like.
Now click the little "f" button on the bottom of the layers palette, and go to "Bevel and Emboss".
Now put these values in.
